Person-centred individual planning

The two-day course is for you who are involved in individual planning and are looking for ways to give this activity a deeper meaning and significance for strengthening the social inclusion of disadvantaged people. Person-centred planning, which we can work on together for two days, includes a range of tools that you will really enjoy doing individual planning with. Plus, it will make a positive difference to the lives of the people you provide help and support to.

For whom the course is intended

The course is designed especially for key workers and social workers who are involved in or preparing for individual planning.

What you will take away from the course

The course includes interpretation and practical training in the use of person-centred planning tools. By completing it you will gain:

  • A deeper understanding of the individual planning process and the interconnectedness of the parts of the individual plan.
  • Understanding the concept of Person-Centred Planning.
  • Practical tools for the concept of Person-Centred Planning.
  • Skill to prepare a One Page Profile and understand its use.
  • Ability to work with and evaluate goals in individual plans.
  • An idea of the nature and use of PATH and MAPs approaches.

You will also take away learning materials, smart templates for practical use and links to other study materials.

Teaching content

  • The principles and essence of individual planning and the value foundations of the concept of Person-Centred Planning.
  • Selected key tools of Person-Centred Planning.
  • Person-centred approaches in needs mapping, including working with the preferences of the people you are planning for and with.
  • Preparing a one-page profile and its practical use.
  • Structured reflection on information and its use in planning, including the formulation of collaborative objectives.
  • Planning steps and evaluating plans.
  • PATH and MAPs approaches, their purpose, structure and practical use.

Scope and form of training, number of participants

The course is held in full-time form in the scope of 16 teaching hours in two days.

The maximum number of participants is 18.

The course is accredited by the Ministry of Labour and Social Affairs of the Czech Republic, accreditation No.: A2025/0076-SP/PC/PP.

Participants who successfully complete the course will receive a certificate of completion.
